Certificates and Licenses are issued in Village Hall by Bronxville's Village Clerk and Registrar of Vital Statistics.  Some of the services are available through the mail.  Please read the details below carefully and follow all directions.  If you have any questions please call the Village Clerk or Registrar at (914) 337-6500.

Through the Village Clerk Bronxville residents may obtain dog licenses and permanent or temporary handicapped parking permits.  The clerk also issues local permits and licenses for gardeners, mechanical amusement devices and taxi drivers.  You may apply in person or through the mail to the Registrar of Vital Statistics for birth and death certificates for anyone who was born or died  in Lawrence Hospital.  

 

Birth Certificates

Who is eligible

a person with a New York State Court Order

the person named on the birth certificate; if 18 years of age or older

the parents of the person named on the birth certificate

the lawful representative of the person named or the parents of the person named on the birth certificate

the Commissioner of Health

a municipal, state or federal agency when needed for official purposes

Applying in Person

go to Village Hall, 200 Pondfield Road, Bronxville, NY  10708

complete and sign the application form provided by the Department of Health (DOH-296A).

present driver's license 

the applicant must be eligible to receive a copy

$10.00 payment per copy

Request by Mail

enclose a check or money order in the amount of $10.00 for each copy

must be on a signed application (DOH-296A) or a letter containing the following information:  name, date of birth, place of birth, father's first and last names and mother's first and maiden names of the person named on the birth certificate; and

the applicant provides a copy of his/her driver's license with his/her current name and address along with his/her relationship to the person named on the certificate

If the applicant's return address is a Post Office Box, with no street address, or the return address is to a third party, a notarized signed consent of the person named on the birth certificate or other person eligible to obtain the copy is always required.

Death Certificates

Who is eligible

a person with a New York State Court Order issued on a showing of necessity;

the properly identified spouse, parent or child of the deceased;

the lawful representative of the spouse, parent or child of the deceased

a person requiring the record for a documented legal right or claim

a person requiring the record for a documented medical need

a municipal, state or federal agency when needed for official purposes.

Applying in Person

applicant must provide the decedent's name and date of death

application form DOH-294A must be completed and signed

some form of acceptable identification (if you're not sure what is acceptable call (914)337-6500 and ask the registrar)

$10.00 payment per copy

Applying by Mail

enclose a check or money order in the amount of $10.00 for each copy

completed Application to Local Registrar for Copy of Death Record (Form DOH-294A)

a copy of some form of acceptable identification with the same last name as the decedent as described on application form

if the applicant's last name is not the same as the decedent, please call (914) 337-6500 and ask the registrar for specific details

Licenses

Amusement Licenses

If you have a manually operated or mechanical amusement device on your premises located within the boundaries of the Village of Bronxville you must obtain a license through the Village Clerk at Village Hall or call (914) 337-6500.

Dog Licenses

Every dog, regardless of age, owned or harbored in New York State for longer than 30 days must be licensed in the town or city where the dog is being harbored. The following are the only license exemptions:

• If the dog is under six months of age and not running-at-large (off the owner's premises)

• Dogs harbored in New York City (these dogs must be licensed by the NYC Department of Health)

• Dogs confined to the premises of any public or private hospital, research institution or a Class A Dealer with a certificate of exemption from the Commissioner

Hunting and Fishing (New York State Sporting License) These licenses are not issued through the Village.  You can click the link below to access information from the New York State DEC website.

Marriage Licenses

The Village DOES NOT issue marriage licenses.  You can obtain one at any TOWN or CITY. 

Taxi Driver License

Any person operating a taxicab in the Village of Bronxville shall have in force and effect a taxicab license issued under the direction of the Chief of Police.  You must contact the police department at (914) 337-0500 to obtain information.
